Purpose of this Article: This solution article provides information on how to view a summary of your benefits and dependent information under the Benefits Summary tile.

You as an employee of the City & County of San Francisco, Superior Court, Community College District or Unified School District can access the Benefits Summary  tile from your My Health Benefits homepage. This tile takes you to your Benefits Summary page, where you can access your benefits and dependent information.

1. Enter a date in the My Benefits on field and click the Refresh button to view your benefits information effective the date that you entered.


2. You can view your benefits information in a card view as in the image above, or switch to a table view as in the image below.





3. Click the type of benefit you wish to review.


4. The details of the benefit type you've selected will appear on another page, which also includes a list of dependent covered under the plan.


5. Health Service System's contact information appears on the upper right of your screen in case you have any questions regarding your benefits information.


6. A Navigation Collection appears on the left side of your page in case you'd like to access other pages.

  • Life Events - There is also a Life Events tile available on your My Health Benefits homepage.
  • Form 1095-C Consent - The intended purpose of this page is to enable you to provide consent to receive your Form 1095-C in an electronic form. However, please note that this feature is not currently available, and all employees receive their Form 1095-C hard copies in the mail.
