Purpose of this Article: This solution article provides instructions on how to request a reisssue of your W-2 form.

Active employees should follow the instructions in Section 1 below to request the reissue of W-2. For Retirees and former employees, please[ skip to Section 2 below.
Section 1:
Step 1. From the SF My Hub Pay tile, click My Pay
Step 2: Click W-2 Reissue Request
Step 3: Select the Tax Year from the search icon if you are looking for a form that is two years or older. It defaults to the previous year.
Step 4: Click Submit
Step 5: You will be asked to verify your identity by entering your City-Wide Active Directory (CWAD) password.
Step 6: Once you have entered your password, click Continue. If you receive a notification stating your password is invalid, enter your password again. If you experience password issues, please click the MyApps Portal tile from SF | My Portal to change your password and return to SF My Pay to request your W-2. If the issue persists, please contact your department IT unit to seek their assistance with your City-Wide Active Directory (CWAD) password.
*If the Verify Identity window does not load, make sure your browser settings allow cross-site tracking, cookies, pop-ups and redirects as well as clear your cache. Your local department MIS/IT Support team can also assist you if you need additional assistance.
Step 8. Click Ok to close the Confirmation
Section 2: For Retirees and Former Employees, send them the attached PDF copy of the W-2 Reissue Request Form since they do not have access to Self-Service option or access to view this solution article.
Step 1. Fill out the employee ID also known as the Disaster Service Worker (DSW) number, full name (Last, First, Middle Initial format), three-character department acronym, and social security number.
Step 1.1. Former City and County of San Francisco employees pay $20.00 per tax year needed by a money order available from the U.S. Post Office or personal bank check made out to the City and County of San Francisco.
Step 1.2. Bring the completed form when picking up the reissued form in person and present the current State Issued ID or passport to Payroll Division staff when picking it up at the address: Office of the Controller Payroll Division 1 Dr. Carlton B. Goodlett Pl., Rm 488 San Francisco, CA. 94102
Step 1.3. List the address, city, state, and zip code if you prefer to receive the reissued W-2 form by mail instead of picking it up.
Step 1.4. Specify the tax year(s) you need W-2 forms reissued.
Step 1.5. Sign and date the form.
