Follow the instructions below to request a reissue of your W-2 form.


Step 1. Fill out your employee ID also known as your Disaster Service Worker (DSW) number, full name (Last, First, Middle Initial format), three-character department acronym, and social security number.


Step 1.1. Former City and County of San Francisco employees, please pay $20.00 per tax year needed by a money order available from the U.S. Post Office or your bank made out to the City and County of San Francisco. 

For active City and County of San Francisco employees, please pay $20.00 per tax year needed by money order or personal check to the City and County of San Francisco. 


Step 1.2. Bring the completed form when picking up the reissued form in person and present your current State Issued ID, City and County of San Francisco ID, or passport to Payroll Division staff when picking it up at the address below:

Office of the Controller Payroll Division 1 Dr. Carlton B. Goodlett Pl., Rm 488 San Francisco, CA. 94102 


Step 1.3. List the address, city, state, and zip code if you prefer to receive the reissued W-2 form by mail instead of picking it up.


Step 1.4. Specify the tax year(s) you need W-2 forms reissued.


Step 1.5. Sign and date the form.
