Provides step-by-step instructions on how to view your W-2/W-2c form


Follow the instructions below to view, download, and print your W-2/W-2c form.


Step 1. From the My Pay tile, click W-2/W-2c

Step 2: Click View W-2/W-2c Forms

Note: Do not use the Sort icon because you can not use the sort functionality to resort the list since it only displays one year. 

Step 3: Select the Tax Year from the drop down menu if you are looking for a form that is two years or older. It defaults to the previous year. 

Step 4: Click View Form button

This form is required to be printed and attached to your Federal and/or State income tax return to access the Notice to Employees and Instructions to Employees, click on the information icon under the Filing Instructions column. 


Step 5: You will be asked to verify your identity by entering your City-Wide Active Directory (CWAD) password

Step 6: Once you have entered your password, click Continue. If you receive a notification stating your password is invalid, enter your password again.  If you experience password issues, please click the MyApps Portal tile from SF | My Portal to change your password and return to SF My Hub to access W-2. If the issue persists, please contact your department IT unit to seek their assistance with your City-Wide Active Directory (CWAD) password.

*If the Verify Identity window does not load, make sure your browser settings allow cross-site tracking, cookies, pop-ups and redirects as well as clear your cache. Your local department MIS/IT Support team can also assist you if you need additional assistance.

Step 8. Your W-2 will appear in a new tab. Click on the Save icon to download a copy or the Print Icon to print a copy.
